just wrote the following, nothing fancy, nothing special, just honest and to the point :) feel free to use it
Dear Sir or Madam:
On 13/12/2003, myself and two members of my party, were booked on flight BD586 (11:05 London Heathrow to Manchester) and the return, flight BD593 at 18:55. This flight was delayed then eventually cancelled, we were put on a British Airways flight, scheduled to leave at 13:45, this eventually took off at 15:15, unfortunately for us, this was far to late for our appointment, in Manchester at 3pm! Consequently we never travelled. I went to the BMI desk, where the lady gave me a reference number of ZH8WY8, to prove we hadn’t travelled, but had indeed checked in.
Three adults were travelling at a cost of £51.50 each, I would of course like to be refunded the money for this most unfortunate of adventures, I was very distressed to miss my meeting in Manchester.
Yours truly,
nocoat

Dear BMI Refund dept,
Thank you for my repayment of the airport tax. However, having spoken to other members of our party on the same flights, i have been informed that they have been refunded in full without question. Please could you advise me as to why i have not been fully refunded in this matter.

Flight BD586 to Manchester 13th Dec 2003
Dear Sir,
I refer to the above flight which was cancelled due to 'adverse weather conditions' despite that it was the only flight to Manchester or anywhere else in the UK that was cancelled on that date.
I booked 4 return tickets on the said flight in the names of myself (James Finnigan), John Finnigan, William Lambert and Daniel Spriggs.
Once the flight was cancelled and the BA flight we were transferred to was delayed by over half an hour, we were too late to attend the event we were travelling for.
We returned to the BMI desk to ask for a refund and were informed that as the cancellation was due to weather conditions, we were not entitled, end of story.
Until this point, I have grudgingly accepted the word of your customer service agent. However, I have been informed that a number of people booked on the same flight have since received refunds.
I will be therefore be grateful to receive a full refund for these tickets at your earliest convenience.
Yours faithfully
Jim Finnigan
